2017-18 Pre-participation Paperwork is now available for All Potential Student-Athletes

If you are interested in playing a varsity or club sport at Framingham State University during the 2017-2018 academic year, there are several forms you need to fill out before you may begin participating. The NCAA Registration and Sports Information Questionnaire are online forms, while the Athletic Training Medical Paperwork may be completed online and emailed using the fillable PDFs or printed and mailed/faxed back to the Athletic Training Office. The Haven and AlcoholEDU portion is a two-part online module.

Every student-athlete should complete the following sets of forms unless otherwise indicated - 

  1. NCAA Registration (for all new varsity sport athletes only)
  2. Sports Information Questionnaire (all varsity and club sports)
  3. Athletic Training Medical Paperwork (all varsity and club sports)

For those student-athletes wishing to play a fall sport (men's soccer, women's soccer, field hockey, football, men's cross country, women's cross country and women's volleyball), these forms need to be completed before you move in for pre-season camp in August. Forms for fall student-athletes are due by August 1st.

For those student-athletes wishing to play a spring sport whose non-traditional season is in the fall (baseball, softball, women's lacrosse), these forms need to be completed by August 1st. 

Winter student-athletes (men's basketball, women's basketball, and ice hockey) should have these forms completed by September 15th.

For those wishing to play a club sport in the fall, paperwork should be completed by September 1st.
