2013-2014 NCAA Student-Athlete Registration is now open

If you are interested in playing a varsity sport at Framingham State University this year, there are several forms you need to fill out before you may begin practicing. This year all of the forms – with the exception of the medical forms (physical, medical history, proof of insurance) – are being done electronically. You have direct access to the athletic training forms through the Athletic Training section of the Athletics website, but in order to access the NCAA-mandated forms, you need to fill this short form out so that you can be activated in the NCAA system. Once you have been activated, you will receive an email with a link to the NCAA forms.

Please note that you cannot fill out the NCAA forms until you have been registered in the system as each student-athlete is assigned a different code. Forwarding the email to anyone who hasn’t been registered in the system will not work for those recipients. 

Please fill out the registration form only ONCE.  Filling the form out multiple times may slow down the response time.

Student-athletes wishing to participate in club sports – cheerleading, men’s lacrosse, women’s rugby and men’s rugby - do not need to complete the NCAA registration form, but do need to complete all other forms.

Please note that prior to being able to try out for any varsity team the following items need to be completed:

  • Pre-Participation Medical and Injury History Questionnaire
  • Informed Consent/Equipment Warning
  • Notice of Privacy Practices
  • Authorization Policy Form
  • Acknowledgement of Insurance Requirements
  • Emergency Contact & Insurance Forms
  • NCAA Paperwork
  • Sports Information Questionnaire


For those student-athletes wishing to play a fall sport (men’s soccer, women’s soccer, field hockey, football, men's cross country, women's cross country and women’s volleyball), these forms need to be completed before you move in for pre-season camp. 

For those student-athletes wishing to play a spring sport whose non-traditional season is in the fall (baseball, softball, women’s lacrosse), these forms need to be completed by September 1. 

Winter student-athletes (men’s basketball, women’s basketball, and ice hockey) should have these forms completed by the first day of classes.
